Perception on accreditation: hospital may-opt not to be accredited during re-accreditation status as long as they maintain the quality standard and the steering committee for QMS is employed and SOPs of each department is implemented. Patient-centred care management includes physicians actively following up on patients on what happening after initial consultation and treatment

ROJoson Patient-centered Care Management includes my actively following-up on patients on what happening after initial consultation with me and treatment by me. 

I am concerned with at least the following: 1) whether the patient is recovering well from the initial consultation and treatment; 2) whether I need to do some adjustment in my diagnosis and treatment inclusive of medications; 3) what is the progress in the resolution of the health problem after seeing me; 4) whether the patient has other concerns; 5) whether the patient has seen a physician and lab clinic that I referred to and what are the outcomes; 6) whether the patient has problems looking for right physician and lab clinic to help me resolve the health problem at hand; and 7) to know what鈥檚 happening to the patient if she has not return for follow-up / check-up with me.

I do the follow-up either through telemedical consultation or through face-to-face medical consultation.  I do the non-face-to-face medical consultation follow-up usually through Facebook messenger, Viber, or SMS (text).

In the follow-up, I also reinforce my advice to them and at times, I give them my writings for more in-depth understanding of my explanation during the face-to-face consultation.

Examples of documentary evidences of my follow-up on my patients after initial consultation and treatment:

After an operation, I do telemedical rounds and check patient鈥檚 wounds, wound drainage, level of pain, medications, etc. through video chat.  Then, I act accordingly.  I reinforce what I told the patients before and right after the operation.

If I don鈥檛 hear from my patients after the initial consultation and I know they should be doing something for their own health, I message them either through FB messenger, Viber, and SMS and ask 鈥淜umusta na?鈥

If I don鈥檛 hear from my patients after the initial consultation and I know they are being treated by other physicians either through my referrals or otherwise, I message them either through FB messenger, Viber, and SMS and ask 鈥淜umusta na?鈥



Asking a patient to send me through viber picture of her wound a few days after mastectomy for me to check.


Asking a patient who underwent mastectomy to send me her monitoring sheet of the tube drainage output

I vibered this patient who was undergoing radiotherapy in a private hospital in which I am not affiliated with and asked her: “kumusta na?” She started her radiotherapy a month ago.


I told the relatives that after their consultation with me, I will look for a colorectal surgeon in Cabanatuan City who could take care of their patient. I asked around and then I messaged them.


Asking a patient with lung cancer for an update on her treatment by another medical oncologist:


For the terminally-ill patient, I follow-up on the status and console the relatives.

Patient-centered care management includes assisting patients in their document requirements and facilitating account settlement

Patient-centered care management is conventionally defined as the practice of caring for patients (and their relatives) in ways that are meaningful and valuable to the individual patient.

It conventionally includes listening to, informing and involving patients in their medical care, note, medical care, which includes medical diagnostics and medical treatment. It usually does not include ANCILLARY support to the medical care which is usually left to the patient and relatives to accomplish on their own.

In the past I have written the following related topics:

As I have said:

Patient-centered care management should not be confined to listening to, informing and involving patients in their medical care. It can and should go beyond this. If needed by a patient, a physician may and must (if within his capability) provide ancillary support.

Doing patient-centered care management is also doing compassionate patient management. My operational concept or definition of a compassionate physician is one who feels and shows sympathy and concern for patients especially those in need of one.

All physicians are expected to be compassionate to patients. The deeper the sympathy to patients, the stronger is the desire to alleviate. The stronger the desire to alleviate, the greater and heavier the assistance to give. Ultimately, it is the assessment of the magnitude and weight of assistance given that determines the degree of compassion.


Here I will add two more inclusions on the ancillary services / support in the patient-centered care management.

ASSISTING PATIENTS AND RELATIVES IN THEIR DOCUMENT REQUIREMENTS

From actual experience up to now, patients and relatives oftentimes, not to say always, complain of difficulty (including delay) in getting the necessary documents prior to discharge from hospital such as discharge summary, copies of lab results, etc. Also, they have difficulty in getting the needed documents after discharge for use in their insurances, SSS, and GSIS requirements such as medical abstracts, medical certificates, etc.

Patient-centered care management should take note of these common problems being faced by patients and relatives in getting their documents from the hospitals. These problems should be mitigated if not totally avoided. On a personal level, I am wary of these and have been trying to help. If need be, I call up the concerned staff to prepare the documents as quickly and as soon as possible.

FACILITATING ACCOUNT SETTLEMENT OF PATIENTS ON DISCHARGE FROM HOSPITALS

Again, from actual experiences up to now, patients and relatives oftentimes, not to say always, complain of difficulty (including delay) in settling their hospital bills. They have the money to pay but there is a long waiting time which can be caused by a lot of factors such as no discharge order from physicians yet; no posting of professional fees by private attending physicians yet; no discharge summary yet; etc.

Patient-centered care management should take note of these common problems being faced by patients and relatives in settling their hospital bills. These problems should be mitigated if not totally avoided. On a personal level, I am wary of these and have been trying to help. If need be, I call up the concerned staff to speed up the processing of hospital bills.

  1. Defined and implemented Corporate Social Responsibility Program 
  2. Preventive and curative projects in the community. It must include measurable outcomes. Examples- HIV/AIDS, CANCER, TB awareness/ prevention camps, 鈥渙peration cataract鈥, etc.
  3. Undertakes 鈥淕reen鈥 initiatives to preserve natural resources
  4. Regular and sustained mass voluntary blood donation activities
  5. Implementation of RA 10932 or 鈥淭he Anti-Hospital Deposit Law鈥
  6. Implements RA 9439 or 鈥淎n Act Prohibiting the Detention of Patients in Hospitals and Medical Clinics on Grounds of Non-payment of Hospital Bills or Medical Expenses” 
  7. For private hospitals, allocation of 10% of its ABC to charity patients while for government hospital, 90% of their ABC

5.  Quality Improvements:

  1. Studies, researches, or projects that aim to improve service and patient outcomes 
  2. Formal and collaborative evaluation of care using analysis of process and outcome data
  3. Documented program for reducing mortality/morbidity rate, caesarian cases rate and others
  4. Innovative programs/projects/equipment to improve preventive or curative process
  5. Compliance to Health Facility Price Advisory Mobile Application (HFPA) to ensure readily accessible to the up-to-date information regarding the prices of health services, and all goods and services being offered.
  6. New technology or any innovation on the scheduling of hospital appointments or virtual consultations.

In my class on Systems Thinking for MHA HA 202 last March 6, 2023, I had a student who posted this question: (delighted to be asked this question)

It is said that hospitals are the most difficult organizations to manage administratively.

Hospital managers would ask me: Is there a way or a secret to make the administration of hospitals simpler or lighter? Or a more realistic question: Is there a more efficient way in hospital administration and one that will ensure performance excellence?

At the moment, as a starter, to coach hospital managers, I thought of three things that can make hospital administration more efficient. With my past experience (29 years 鈥 1989 to 2018) as hospital and quality manager, I have come to think, perceive, opine and recommend (TPOR) the use of three frameworks and practices as foundations in managing hospitals 鈥 foundations to establish at the start and then develop and eventually master and to use them all constantly and throughout the life course of the hospitals and which I think can contribute to efficiency in hospital administration 鈥 foundations once mastered will lead to performance excellence.

These three foundational frameworks and practices are systems-based practice; departmental design and development framework; and hospital quality and safe management system framework.

I am Geraldine XXXX, an MHA student in my first year, 2nd semester.  I want to apologize to you for I was not able to actively engage in the discussion on Systems Thinking in Hospitals with Focus on Departments, which you gave last March 6, 2023.  We recently moved to a new home and our internet signal is not yet stable.  It is also for this reason that I was only able to answer the OLETE yesterday.

Thank you for providing us with the recorded lecture.  Professor Kathy ZZZZ gave us a background on Health Systems Thinking and Complexities under HPAd 201 last semester and you enlightened me further on this concept.  If I may indulge, I have a few questions in mind as follows:

1.  Am I correct to believe that Systems Thinking should be the foundational knowledge that every leader/manager must learn and fully understand?  I think the framework you developed encapsulates everything there is about Leadership and Management.

2.  Based on your experience, what are the effective strategies to deal with the elephant in the room?

For our individual assignment, I am interested in tackling the Equipment/Material Program.  As an actively practicing pathologist and as Ancillary Services Director at the same time, I am involved in equipment/material acquisition and management.  May I ask your approval if this is alright with you?  Your opinion is highly regarded.

As to the second question – 

2.  Based on your experience, what are the effective strategies to deal with the elephant in the room?

When you say “elephant in the room” – I suppose you mean there is a “problem” or ‘problems” whatever it is or they are, obviously present in the hospital or in the department, but it or they are being ignored or not subjected to scrutiny, problem-solving and solution.   This situation is common particularly in government hospitals.

Your question is “what are the effective strategies” to deal with the elephant in the room?

First requirement is to have a competent leader and manager with a long-term vision for the excellent performance of the hospital and its departments and services.

Second and an ancillary requirement to the first requirement in the competency qualification is to have competencies in the following:

– systems thinking or perspectives in problem-solving and decision-making not only confined to the specific elephant in the room but all-encompassing of all current and potential problems and challenges in the hospital / departments.

– problem-solving skills inclusive of cause analysis and formulation of effective, efficient, just and compassionate solutions

– skills in people management particularly in using participatory management approach or style that promotes motivation, alignment, cooperation, collaboration and integration

– discipline and perseverance in continual improvement in all the management systems of the hospital / departments

– Innovation and creativity in problem-solving and decision-solution-making for all the elephants in the room and in promoting sustainable performance excellence for the hospital / departments 

The nitty-gritty and details of the processes of the abovementioned strategies have to be spelled out and I am sorry I cannot provide them at this time.

I am part of the administrative staff that handles licenses and accreditation, am surprised with this new insight on other accreditations aside from the usual ISO. I do get the point of the setting the standards high, though there’s always this thought that once a hospital such accreditation, would they make sure to keep it up or until the next renewal accreditation na lang. Just my two cents po.
